Contractual case management for children/youth
Provide in home placement stabilization case management. Conflict resolution skill, ability to defuse crises, familiarity with McHenry county service system required. SASS Therapist
Be a part of a team to help children and their families in crisis. Provide clinical assessment, crisis intervention, case management, individual and family counseling for children and adolescents, and their families for a McHenry County home and community based program. E.O.E.
Requirements:
Master’s degree in counseling, social work, or related field.
A valid driver’s license.
Experience working with youth and their families.
Job Responsibilities:
- Provide intensive home and community based services to children and adolescents and their families.
- Monitor psychiatric hospitalizations and provide discharge linkage and after-care.
- Develop comprehensive mental health assessments and treatment planning.
- Provide therapy, counseling, and case management services for clients and families.
- Maintain current accurate record of services and submit paperwork on a timely basis.
- Majority of services provided in the homes, schools, and community locations.

Community Support Specialists:
(Formerly called Mentors) Specialists should be available to work with each youth up to four hours per week. Ideally, each Specialist will have a minimum of two youth assigned at all times and therefore will deliver 8 hours of service per week. Training and supervision are provided. Specialists must pass a background check and a urine screen test as part of the application process. Specialists must be at least 21 years of age and pass all background requirements before being assigned to cases.
- Provide assistance to clients in achieving and maintaining rehabilitation, resiliency, and recovery from mental disorders.
- Facilitate skill building, identification and use of natural supports, illness self-management, and use of community resources.
- Assist in skill building to develop functional, interpersonal, family, coping, and community living skills.
